Yes. Testosterone, HRT, and weight-loss medications for your primary track are included in the Hormone & Weight Loss Program's $149/month, and in Concierge Primary Care's $329/month flat rate. GLP-1 medications are billed separately at cash price on both programs.

Yes, anytime. Many members start with the Hormone & Weight Loss Program and add a second track or upgrade to Concierge Primary Care as their needs grow.

The Hormone & Weight Loss Program is available by telehealth anywhere in Arizona. Concierge Primary Care — including in-home visits — is available to patients in Flagstaff and the surrounding area, within about a 25-minute drive.

Yes. Mend memberships and most associated medical expenses are HSA/FSA eligible.

Concierge Primary Care is intentionally capped so Shawn can maintain a small panel and know each patient's biology deeply. Once full, we maintain a waitlist.

Both programs are month-to-month — cancel anytime, no penalties.

Once you're stable, most patients step down to Maintenance at $99/month, which covers ongoing prescriptions, messaging, and an annual deep dive. Ask Shawn if you're a candidate.
